Dear Parents, you can now download our revised Parental Agreement for the School to Administer Medicine form by clicking on the below link or visiting the 'menus and forms' drop down box under the 'Office' tab on our website homepage. 

Alternatively, the forms can be collected from the school front office in the usual way.
